Made by heating glazier's bench up above 5000 degrees (see comments below) and selecting "Fuse silver powder to sheet glass" from the bench's menu. If the temperature of the bench is too low, the powder "blows away" and is lost.
There is a minimum temperature above which the powder will fuse to the glass. More research is necessary to determine the actual temperature. The 3400 degree value is from my first successful attempt, and I know that 1900 degrees is too low. -- Mnhtp

I was somewhat annoyed to discover that the first mirror I made at 4500 worked fine, the second one, straight afterwards (starting at 4550) failed... There must be some element to this that we don't know about. - Preladon

The ending temp was about 4500 as well - I added 2 more charcoal. I was so infuriated about this, because it seemed like a bug, that I paged a GM. He then explained (since I hadn't considered the random factor) that he'd never had it fail keeping the bench >5000. Perhaps I was just really unlucky with the second mirror, and the chance of failure is proportional to (5000 - current heat)? This would explain why some people have had success as low as 3400, but others have had failure >4500 (but less than 5000).
